Asthma Action Plan:  All people with asthma should have an asthma action plan. An asthma action plan (also called a management plan) is a written plan that you develop with your provider to help control your asthma.  The AAP is available in many formats and languages, as well as different sized fonts and graphics.

  • CDC’s Asthma Action Plan includes print, electronic, and computer based plans in English and Spanish
  • The American Lung Association’s Asthma Action Plan is available in different font sizes in English and Spanish

YES WE CAN Children’s Asthma Program, Centers for Disease Control and Prevention:This toolkit provides manuals and resources to start a community based asthma program based on the medical social model of care with emphasis of the community health worker as part of the health care team.

Asthma Control Test, GlaxoSmithKline: is a patient-based tool for identifying patients with poorly controlled asthma.
